Toenail Health Self-Assessment: What's Wrong With My Nail?

An interactive guide to help you differentiate between fungus, trauma, and complex nail disorders.

Medically Reviewed By Dr. Sarah Jenkins, DPM Chair, Medical Review Board

"Not sure what's going on with your toenail? Let's figure it out."

Answer these clinical questions to understand the most likely cause of your symptoms and the next logical action for your health.

Q1

Is there persistent pain or throbbing?

If YES:

This is likely an Ingrown Toenail or a secondary infection. Pain is rarely a symptom of early-stage fungus.

If NO:

If it's just a visual change without pain, move to the next question.

Q2

What is the primary color change?

Yellow/Brown:

Highly indicative of Nail Fungus (Onychomycosis). Check for thickening.

Black/Purple:

Typically Trauma (blood under nail). If it doesn't move as the nail grows, see us.

White Spots:

Can be "White Superficial Fungus" or just minor trauma from shoes (Leukonychia).

Q3

Is the nail getting thicker or "crashing"?

Thickening (Hyperkeratosis):

Common in fungus, but also a normal sign of aging or nail psoriasis.

Inward Curving:

This is likely Pincer Nails. This requires medical bracing to prevent extreme pain.

Still Not Sure?

Nail conditions are notoriously difficult to self-diagnose because many symptoms overlap. The most effective "conversion tool" is a professional assessment.

Patient Frequently Asked Questions

Q Should I see a podiatrist or a dermatologist?

Both can treat nails, but podiatrists specialize in the mechanical and surgical aspects of the feet. If you have pain, ingrowns, or need laser treatment, a podiatrist is usually the better clinical choice.

Q Can I send a photo of my nail for diagnosis?

While a photo can provide a preliminary idea, a definitive diagnosis of fungus requires a physical exam and sometimes a small "clipping" to be tested in a lab to identify the specific pathogen.
